Constant adjustments, based on feedback, are necessary. This becomes an issue 1) when management wants to release a plan and then forget about it and 2) feedback, especially when negative, isn’t welcome.

Feedback is critical because, according to Sidney Yoshida, the author of the Iceberg of Ignorance concept, top managers know about 4% of an organisation’s problems.
